Hi,
You need to qualify 1st and 3rd pqr as suggested by Rameshji. But you can avoid 2nd pqr by using asme interpretations and still qualify the entire range of 3-50mm.

As a first step, refer Table QW-451.1 in ASME Sec IX for PQR thickness limits. 

Based on some Essential / supplementary variables this Minimum or Maximum thickness will vary from this table.

For example, refer QW-256 -GTAW welding variable table. u can see QW403.6 , QW 407.4  i.e T limits row and study relevant QW clause which is self explanatory

Your case

Q1) What will be the thickness range qualified is it 10.97 to 16.97 or 5 mm to 16.97 or 5 mm to 21.94.

Since your PQR have impact requirement i.e supplementary variable requirements , Your PQR thickness range qualified is 10.97 to 21.94

Q2) If i need to qualify a WPS with Thk ranges 5 mm to 25mm which thickness need to be selected .

Qualify one PQR (1) with 6 mm thick, this will qualify 3 to 12 mm thk-
 this pqr needs to be qualified

Qualify other PQR (2) with 12 mm thick , this will qualify 12 to 24 mm thk - 
this pqr can be avoided by using asme interpretations

Qualify other PQR (3) with 25 mm thick, thick will qualify 16 to 50 mm thk- 
this needs to be qualified

So you will have WPS combining all PQRs with 3 to 50 mm which almost covers all your Fabrication requirement for CS pipes.
