Since You mentioned with impact test, i have suggested 3 PQR thickness. 

if PQR (2) absent, then there is thickness gap between 12 to 16 mm.. 

Mr. Prashant Panchal or any experts shall provide code case and explanation how / why PQR (2) is not required when impact test calls far for our understanding pl. 

If impact not required u can use that PQR (3) 25mm thk , which qualifies 5 to 50 mm ( with out PWHT)

Q1) What will be the thickness range qualified is it 10.97 to 16.97 or 5 mm to 16.97 or 5 mm to 21.94.

Q2) If i need to qualify a WPS with Thk ranges 5 mm to 25mm which thickness need to be selected .
