I want to qualify procedure according to ASME Sec V, but i don't now defects must be machined into the weld or
natural.if natural how to be compared the real size with assesssed size?

It depends what type of defects you want to check.
Like porosity - you can use a wet electrode or long arc
Cracks - Embed Copper wires on carbon steel plates and weld
Lack of Fusion - Ask your welder to not to fuse the side wall of the Joint
