It looks like this failure is HAZ and this mettalurgical failure is due to increase in material hardness due to welding ; this failure looks like this is happened in service ; normaly these type of defect comes in service when these section subject to pressure. since this failure is on SS 312 - 304H which require to be analyzed ; i will suggest you to measure teh hardness value of this material near to crack area and also to measure one reading at 100mm from
