Dear  Mr. Karandish,
 
The crack appears to be over the entire weld, and obviously, the welding process needs to be corrected. I feel that this requires a thourough
investigation at the site and metallurgical evaluation and testing if we need to correct this problem. I am a Metallurgy/ welding consultant ( IIT Kharagpur, Metallurgy, 43 years experience) with experience in welding of S.S., P91 steel, etc. I am willing to take up this problem.

Dear experts,
In attached images you will find the failure happened on the junctions of header to heater tubes of a natural gas + steam heater. The heater specification is as follows:
Inside fluid: natural gas + steam
Outside fluid : flue gas from burning of natural gas in the reformer of an Ammonia plant.
Tube and header material : SA-312 TP 3047H
Considering the above let me have your suggestion regarding the failure reasoning.
