A vessel is being fabricated as per ASME SEC VIII DIV 1.
 
A temporary blind flange fabricated from plate is used for hydrotest. Hydrotest pressure is 3003 PSIG.
 
During hydrotest it was observed that contractor has welded two plates to obtain required thcikness for temporary blind flange.Please refer attached photo.
 
Whether practise of acheving required thickness of blind flange by welding 2 or more plate is safe and acceptable.
 
please give code reference.
