Incomplete fusion in a GTAW weld is usually caused by the welder having 'poor technique'.
1st. He was not dwelling long enough in the side walls. When the arc / weld puddle moves into the side wall it literally washes out, or 'cuts' out a small amount of the side wall. The welder must hesitate there to allow the 'weld puddle' to fill in the washed out area.
2nd. The welder could be carrying to much metal, or in other words, the weld puddle could be to large and it is 'rolling' over' or not penetrating the previous weld pass in some areas and penetrating in other areas.
3rd. His amperage is to low.
