It is a simple root such back although not to a severe level. The reason Mr. KG had given in a very simple understable way.  If you want to catch this indication in RT, first flush grind the face reinforcement to the pipe surface level and redo the RT and then you can find it. 
You might find other example of such finding in RT e.g. you have small spatters in the root but they will not be visible on the RT as they will got merger with the face reinforcement welds.

for acceptance, better perform a inside VT if possible direct or otherwise with added equipments e.g. boroscope or hand extended mirror if accessible. For accepting, it must be equal or exceeding the pipe internal in general.
