[MW:8840] LOP in MIG Welding Qualification

Dear All,
Recently I have noticed this in one company and want to share with you.
It was a procedure Qualification as per sec IX.
Process is GMAW (MIG).
Plate thickness is 25mm.
Wire Dia is 1.2mm
Root gap is 2.5mm.
Current was 75 A.
After root pass welding found LOP for almost total length.
What is the best way to avoid LOP.
