You have not specified full details. Assuming that it is all round welding and if have not drilled a hole on the pad for gas to escape it is possible that due to expansion of entapped gas between the pad and the shell , cracks might have developed.

I am doing tank construction. I have found some cracks on the pad toe and pad corner. Shell thk 38 mm, pad thk 30 mm and fillet weld size 21 mm. we were controlled everything. (electorde quiver temp. preheat, oven temp. before start welding good cleaning and etc).

Shell mat'l  : A537 
Pad mat'l   : S275 Gr J
 
Electrode    : E7018
 
Pls any one can sugest to me the causes and prevention.

you doesnot specifiy what type of crack that was , We can excute the cause of crack as per the type .

It might be stress concentration .
