I would say that you have an area of underpenetrated root to the right and a mighty area of over-penetration where the welder has burnt through. It is a clear reject but maybe someone else will do a formal assessment to API 1104 criteria.

Please find attached image of a RT film taken on a repaired area on a 24" Diameter and WT 12.7mm.
The original welding process were GTAW Root and SMAW fill and Cap.
How can we classify this defect?
The Acceptance standard for this is API 1104.
Please shed some light on this.
