what is the location of failure, weld, HAZ, BM (which side)? NDE may not detect minute linear flaws, you need run full failure investigation on samples at laboratory,

One LTCS ASME DIV 1 vessel failed in Hydrotest in the joint of 2"LWNRF with Body blind flange. Its a set on type welding.
Nozzle is of SA 350 Gr. LF2 CL1 (16.7 thk) and the body blind flange of SA 765 Gr II (117 thk).
The UT test of the joint shows satisfactory.

Can anyone tell what cud be the possible reasons pls.
