Thanks, Manpreet and others,
It was captured during RTFI and the lengths were 12 mm and 25 mm at 2 different locations. I have already given both as repair. But the contractor came with this table which shows Category M fluid, girth weld, weld imperfection ' Incomplete Pen' is referred to as B. On the following page, B refers to "depth" of IP to between ≤1mm and ≤ 0.2Tw (whichever is lower is implied) and not length. Now, that is one tough task to determine depth of an IP, by RT at least. In Cum Length, < 38 mm in 150 mm, we have acceptance.
So, is my judgement correct or no?

As per ASME B31.3, acceptance criteria for incomplete penetration is:- between ≤1mm ann ≤0.2Tw

was it caught by Radiography or ultrasonic testing?

If UT then there is possibility that what you are referring to "incomplete penetration" might
actually be "Lack of sidewall fusion" for which table 341.3.2 Note 7 states that
"tightly butted unfused root faces are unacceptable".
