[MW:16355] Plugging of incorrectly opened drain nozzle opening at 5Cr-1Mo Shell of Heat Exchanger( 2" 300# - Shell thickness : 26 mm.)

Dear all,

Please advice that if it is applicable to put and butt weld insert plate for the mentioned incorrectly opened nozzle or should we replace the whole plate?

and, any code/spec. reference?
