Dear Shaikh Ahemad,
This is  something which regularly happens in industry .
Various details are missing which are necessary to weigh out the consequences of the decision such as.
1. Engineering application whether it is static or dynamic.
2. Nature of the defect.
3.Machining stage i.e. a.Qualification for next process
b. Pre-machining  c.Stage machining or d.Final machining
c. Under which condition and who has taken the decision.
d.Is there a systematic process and authority specified 
  in the quality manual.
e.What are the actions taken for avoiding recurrence.
f. Costs for processing and rejection at various stages
  including use in service(customer-final user)

   You will be able take decision based on the above details.

As per Our Client Inspection Test Plan , Inspection of Casting is HOLD for Us & Witness for client , but Vendor send casting for Machining without Visual Inspection. Acceptance standard is MSS SP 55 Exmple A.
 
Let me know how to Overcome this problem ?
