[MW:17862] Monthly NDT(RT) statistics by welders (pass and reject rate )

Dear experts Good day to everyone and a happy weekend. I would be grateful if someone can shed some light on the above subject by providing some excel format or any format that you use in your company to monitor the ndt pass or reject/ welder performance which is even good to keep as a permanent record and to know how many % of joints made by a particular welder is passed or failed.
