100% stands for RT for all butt welds
 
Full radiography is defined in UW-11 of ASME Sec. VIII (Not 100%)
 
But the only confusing sentence in code is as mentioned below.
 
UW-52 SPOT EXAMINATION OF WELDED JOINTS
NOTE: Spot radiography in accordance with these rules will not ensure a fabrication product of predetermined quality level throughout. It must be realized that an accepted vessel under these spot radiography rules may still contain defects which might be disclosed on further examination. If all radiographically disclosed weld defects must be eliminated from a vessel, then 100% radiography must be employed.

Full radiography is equivalent to RT-1 stamping as defined in UG-116. As you can see, butt welds in small (not exceeding 10"), thin wall nozzles are not required to be RTed.
100% RT means all butt welds, including those in small nozzles will have radiography examination in full length. This is not a code requirement. It is usually a service requirement. Vessels in wet H2S service often require 100% RT.
Full radiography need not be 100%  See UW 11(a)(5)(b).
