What was the code that was used to qualify the WPS/PQR/WPQ?

Were the PQR/WPQ test specimens tested at an accredited third-party lab? If so, do you still have the test certifications from the third-party lab stating acceptance, with the PQR that was used and other info etc. noted on the test certification(s)?
