generally for small plate attachment .....where the shell is not penetrated is not necessry tale hole....the internal pressure acumulated during welding is quite small.....but as you said the is better as per engineering recomandation
for nozzles reinforcement we do also pneumatic test ....on that tale hole.....

But how about doubler plates and supports which will be permanent attachments on shell. I know tell tale hole is not required for leak test but at least vent hole is reqd as per good engg practice.But API 650 is silent on this issue.
