[MW:35140] comment recive dfrom client

Dear Sir
i have recived a comment from my customer on PQR submitted as our PQR/WPS code edition 2015 is PQR dated 07.02.2019 what is the format or document to show it was used in last year ,we are using same since last 5 year.

 "'It is not allowed to use PQR issued five years before their application for the equipment construction unless VENDOR can demonstrate that the relevant WPS has been used during the last 5 years. The WPQR shall contain a document, which assure the welder activity shall be according to adopted Code"".

Kindly let me know what reply, and pls send me format of  Weld plan, weld map and test plan
