First, you should give us sufficient information such as the AWS Classification, the wire and flux name, and the ASTM name of the material.
second, seemingly your welding speed has been high, I can see the long beads in the welding direction.
third, sticking the slag usually happens when the A is too high and the cause of porosity usually is moisture in the flux, have you baked the flux before use?

1. Reason for such issue to occur. 
2. Is the issue of Flux (neutral basic flux) or Flux cored wire used in the process.
3. Please advise how can this be sorted out.
