Lack of fusion: base metal not melted. Why? Because base metal temperature did not achieve 1510 degC. Why? Because heat input not adequate (forget high or low). You may use  a high heat input welding method (per definition) and you may still get lack of fusion. With GMAW it happens mainly because the welder "runs" too fast in order to achieve high productivity rates. You do not wonder why many EPC specs do not allow GMAW either for pressure equipment or for critical structural steel?

Incomplete fusion is due to less Heat input?? not because of voltage. mainly depend on current and other factor also

voltage depends on Arc length , manual welding ( constant current), it varies with welder to welder and u can preset the voltage before start welding but u can set amps,, but in GMAW, u can preset voltage and separately and Arc length will be always constant because it is Constant Voltage/Potential Power source

How GTAW is lower heat input process in which small dia. Wire use high current with low speed!!

You can do per as good engineering practice this can lead to problem in weld after welding with GTAW on SMAW pass due to high heat input in GTAW..
