Dear friend,
As a reference you can see EN 1043-1. You can practically use it when your spec. or procedure referes to it, but in general for better estimation of hardness behavior in different locations, it is recommended that 3 indentations done along each location that is important for you (e.g. BM,HAZ,WM of CAP on each side or BM,HAZ (containing FL),WM of root on each side or other selected locations).

generally code mentioned base metal, haz, weld metal not mentioned how many locations need to taken from each joint
