Dear,

I havn't ever seen any code to specify the definition of such both terms.
basically,1:3 transition used to be further described for grinding smooth.
smooth is required to avoid the stress concentration.while flush is usually for the requirement of assembly and fit-up.

SMOOTH MEANS - NO SHARP EDGES  LIKE A CURVATURE FINISH
 
GROUND FLUSH  MEANS  -IN CASE OF ORIFICE FLANGES INSIDE  WELD  WE DO GRINDING AND  MATCH WITH BASE METAL THICKNESS

Please differentiate two terminology of ground smooth and ground flush in fabrication.

Hi,  You are correct, Minimum refers to the one cycle for which this part will expose until final work shop PWHT. hence mill test certificate shall include this.  Max.
