Nanesh already picked it, In additon, For PQR qualification ASME sec ix do not compell to perform any volumetric examination such as UT/RT but for welder qualification either UT or RT is required (2010 edition).
 
 Lack of Side wall fusion is perdominate defect generally found in GMAW/FCAW short circuit transfer hence UT examination is recommended for these process (API 577, welding inspection and metallurgy)
 
LOF problems can be solved by proper heat input and vertical uphill method.
