Thanks for your confirmance, yes it is indeed a prject requirement else QW 202.3 would be applicable,

So, A repair PQR with 9.5mm thickness will serve the purpose, Thanks again.

Refer QW 202.3 of ASME Sec IX:
 
You are on right path.
 
Moreover, you need not to have repair PQR unless it is project specific requirement. As per above mentioned clause, you can use any WPS which is quaified for fresh welding and meeting all your all other requirement.
