Refer QW 202.3 of ASME Sec IX:
 
You are on right path.
 
Moreover, you need not to have repair PQR unless it is project specific requirement. As per above mentioned clause, you can use any WPS which is quaified for fresh welding and meeting all your all other requirement.

A repair PQR "1458" prepared with same materials for same 12.7mm base metal thickness, repair WPS "5799-10" for PQR 1458 have base metal thickness of 9.5mm,
 
since Impact test is mandatory, base metal thickness limits shall be 12.7 to 25.4mm, which means WPS is not qualified, Please confirm my understanding.
