I couldn't find the P No. and Gr. No of the material(ASTM A 519 Gr.4130) which you have mentioned in your query.The following ASTM A 519 Grades are listed in ASME BPV SEC IX, 2010 Edition.

Hi,  You are correct, Minimum refers to the one cycle for which this part will expose until final work shop PWHT. hence mill test certificate shall include this.  Max. SPWHT, refers to additional cycles of heat treatment that are reserved for future repairs during equipment lifetime.  Again, these simulated no.
