[MW:7185] problem SS 316 flange to pipe welding

We are fabricating flanged connections for one of our customers
flange dia 350 id220 and thickness is28 mm is joined to 220od
8thk pipe After welding it is observed that out of 6 jobs in 4
jobs the flange has warped in two jobs it is beyond repairs to such a
extent that the face & od to be machined is beyond tolerances (od run
out is 10 mm face is out by 7 mm )
Anticipating this problem we have press fit the components and made
the arrangements for rapid cooling but still this has happened
Now we need to manufacture 10 more flanged connection of the same
type
what precautions should be taken
The welding is done by GTAW 5mm thk fillet all over 5%thoriated 3mm
rod with filler ss 316L 2,5thk current 180Amps inside welding was
fused where as for out side welding filler was given
