Regarding the cold lap issue...

Welder technique is almost certainly the root cause with this defect. Instruct your welders that the puddle must be "led" by the arc "light". 

Also any bumps or lumps left behind on previous passes should be smoothed down at the sidewalls so that the weld metal merges smoothly with the parent material before the next pass is deposited.    

And, the thickness of each deposited pass should not exceed 1/4" or 6 mm.  

Using FCAW, many welders try to deposit too much metal, with improper technique and inadequate preparation and the results are cold lap, slag traps and high hardness.
